Abstract. We examined 22 eyes of 22 patients regarding the changes in the bacterial flora in the conjunctival sac before and after dacryocystorhinostomy(DCR). All the cases showed postoperative improvements in epiphora and fluorescein dye disappearance test. Their age ranged from 48 to 81 years,average 71 years. No case has received antibiotics during 6 months before surgery. All the cases received topical antibiotics for 2 months after surgery to be discontinued thereafter. Bacterial culture was positive in 18 eyes(82%)before surgery and in 8 eyes(36%)3 months after surgery. The difference was significant(p<0.005). This finding seemed to reflect improved self-cleaning effect in the conjunctival sac. Cases who showed positive bacterial flora before and after DCR were characterized by significantly longer duration of epiphora before surgery than the others.
