We performed vitreous surgery using a 25-gauge instrument system on 33 eyes of 31 patients. The series comprised idiopathic epiretinal membrane 26 eyes,secondary epiretinal membrane 3 eyes,and branch retinal vein occlusion 2 eyes. Simultaneous cataract surgery through corneal incision was performed on 27 eyes. The procedure was less efficient than that with 20-gauge system but the outcome was almost the same. One eye developed iris prolapse during insertion of microcannula. Four eyes needed additional scleral suture. Postoperative complications included transient ocular hypotony 7 eyes,transient bleb formation 10 eyes,leakage through the conjunctival wound 4 eyes,and choroidal detachment 4 eyes. Postoperative inflammation or foreign body sensation was almost absent. It was possible to perform minimally invasive surgery with this system.
