　25ゲージシステムによる経結膜的硝子体手術を31例33眼に行った。内訳は特発性黄斑上膜28眼，続発性黄斑上膜3眼，網膜静脈分枝閉塞症2眼である。27眼では角膜切開による白内障手術を同時に行った。硝子体切除の効率は20ゲージシステムより劣ったが，ほぼ同様な手術成績が得られた。カニューレ挿入時に角膜切開創からの虹彩脱出が1眼にあり，4眼で強膜創縫合が必要であった。術後合併症として，一過性低眼圧が7眼，一過性ブレブ形成が10眼，結膜創からの漏出が4眼，脈絡膜剝離が4眼にあった。術後炎症や異物感の訴えはほとんどなく，低侵襲手術をすることが可能であった。

We performed vitreous surgery using a 25-gauge instrument system on 33 eyes of 31 patients. The series comprised idiopathic epiretinal membrane 26 eyes,secondary epiretinal membrane 3 eyes,and branch retinal vein occlusion 2 eyes. Simultaneous cataract surgery through corneal incision was performed on 27 eyes. The procedure was less efficient than that with 20-gauge system but the outcome was almost the same. One eye developed iris prolapse during insertion of microcannula. Four eyes needed additional scleral suture. Postoperative complications included transient ocular hypotony 7 eyes,transient bleb formation 10 eyes,leakage through the conjunctival wound 4 eyes,and choroidal detachment 4 eyes. Postoperative inflammation or foreign body sensation was almost absent. It was possible to perform minimally invasive surgery with this system.
