 A 33-year-old woman was referred to us for optic disc swelling in her left eye. She had had retrobulbar pain in her left eye since one month before. Her corrected visual acuity was 1.0 in either eye. The left visual acuity became 0.02 3 days later associated with papillomacular scotoma. Two sessions of pulsed corticosteroid therapy with methylprednisolone at the dosis of 1,000mg for 3 consecutive days proved futile,resulting in optic atrophy and visual acuity of 0.01. She developed retrobulbar pain in her right eye one year later,followed by disc swelling 2 weeks later. The visual acuity was intact. Pulsed corticosteroid therapy induced decrease in retrobulbar pain and disc swelling. Similar episodes later were controlled by the same treatment. This case illustrates that bilateral optic neuritis may run a contrasting course between the two eyes. Prompt intervention with pulsed corticosteroid appears to have been the major cause for the observed difference.
