虚血性視神経症の治療成績

Therapeutic outcome of ischemic optic neuropathy

　過去7年間の虚血性視神経症の自験例14例16眼を検索した。男7例，女7例で，2例が両眼に発症した。年齢は58～84歳，平均69歳であった。15眼に対し，副腎皮質ステロイド薬と循環改善薬の全身投与，星状神経節ブロックを単独または併用した。視力は9眼（56％）で改善し，7眼（44％）で不変であった。初診時の視野は水平半盲10眼，傍中心暗点3眼，中心暗点2眼などであり，複数回視野検査ができた14眼での視野の転帰は，改善6眼，不変4眼，悪化4眼であった。虚血性視神経症は，薬物などでの治療により，視力，視野が改善することがある。

　We reviewed 16 eyes in 14 cases of ischemic optic neuropathy during the past 7 years. The series comprised 7males and 7 females including bilateral cases. The ages ranged from 58 to 84 years,average 69 years. Fifteen cases were treated by systemic corticosteroid,vasodilator,and/or stellate ganglion block. The visual acuity improved in 9 eyes（56％）and remained unchanged in 7 eyes（44％）. The initial visual field changes included horizontal hemianopia 10 eyes,paracentral scotoma 3 eyes,and central scotoma 2 eyes. Perimetry could be repeated in 14 eyes. The visual field improved in 5 eyes,remained unchanged in 4 eyes,and deteriorated in 4 eyes. The findings show that medication may improve visual acuity and visual field in ischemic optic neuropathy.
