ミカファンギンの全身投与と病巣掻爬術併施で治癒した角膜真菌症の1例

A case of keratomycosis cured by curettage and systemic micafungin

60歳女性が2日前からの左眼霧視で受診した。1か月前から1週間使い捨てソフトコンタクトレンズを使用していた。毛様充血，角膜中央部の潰瘍と実質融解，前房内フィブリン析出が左眼にあった。潰瘍底から採取した検体に胞子状の真菌があったが，培養では真菌と細菌は陰性であった。病巣を掻爬し，酵母様真菌に有効なミカファンギンの全身投与を行った。以後，症状が軽快し，瘢痕治癒した。初診時に手動弁であった左眼矯正視力は，15週後に1.0に回復した。角膜真菌症に対しミカファンギンの全身投与が奏効した症例である。

A 60-year-old female presented with blurring in her left eye since 2 days before. She had been using weekly disposable soft contact lens since one month before. Her left eye showed ciliary injection,infiltration with stromal melting in the central cornea,and fibrinous exudate in the aqueous. Spore-like fungi were found in the scraping from the corneal ulcer. Culture showed no fungal or bacterial organism. She was diagnosed with keratomycosis. We treated her by curettage and systemic micafungin. The corneal lesion immediately started to improve and healed after scarring. Corrected visual acuity in the affected eye was hand motion when first seen by us and improved to 1.0 15 weeks later. This case illustrates that systemic micafungin may be effective for keratomycosis.
