フィプロネクチン点眼を併用した角膜プラーク掻爬術

Ablation of corneal plaque with topical fibronectin

　春季カタルに併発した角膜プラーク11眼に外科的掻爬術を行った。上皮伸展の足場を整え，上皮欠損の再被覆を促進ずる目的で，術中と術後1か月間，患者の血漿から採取したフィプロネクチンを点眼した。角膜上皮欠損は術後2日から13日（平均5.8±3.5日）で消失し，角膜の透明性が向上した。術後合併症は皆無であった。2眼に角膜プラークが再発したが，再手術で治癒した。抗アレルギー治療が無効である角膜プラークに，本治療は有効である。

We surgically treated corneal plaque in 11 eyes with vernal keratoconjunctivitis by debridement. To facilitate epithelial healing, fibronectin was instilled during and for one month after surgery. The corneal defect disappeared 2 to 13 days, average 5.8±3.5 days, after surgery with improved corneal transparency. There was no complication throughout. Corneal plaque recurred in 2 eyes and could be cured by another surgery. Surgical removal of corneal plaque with fibronectin seems to be a treatment of choice for corneal plaque in vernal keratoconjunctivitis.
