濾過胞漏出に対する保存角膜被覆術

Use of preserved cornea in the repair of leaking filtration bleb

目的：難治性濾過胞の漏出に保存角膜による被覆を行った4眼の報告。症例と方法：症例は緑内障4例4眼で，年齢は55～73歳（平均63歳）であった。原因となった前回手術は，42年前のScheie手術1眼，3.5年前と4年前にマイトマイシンC併用の線維柱帯切除術各1眼，48年前の牛眼手術1眼である。濾過胞を切除して保存角膜で被覆し，その上を結膜で覆った。結果：全例に濾過胞様の結膜隆起があったが漏出はなくなった。眼圧は3眼が13～18mmHg，1眼が24mmHgであった。結論：保存角膜を使うことで漏出性濾過胞を処理できた。この方法では術後も濾過機能が残存していると推定される。

Purpose：To describe 4 eyes that were treated by preserved cornea to repair of the leaking filtration bleb. Cases and Method：The present series included 4 eyes of 4 patients. Their age ranged from 55 to 73 years,average 63 years. The previous surgeries comprised：Scheie procedure 42 years before in one eye,mitomycin C-assisted trabeculectomy 3.5 and 4 years in one eye each,and surgery for buphthalmos 48 years before in one eye. The repairing surgery consisted of bleb removal,patching by preserved cornea,and covering by conjunctiva. Results：All the eyes showed focally elevated conjunctiva and no leakage. Final intraocular pressure ranged from 13 to 18 mmHg in 3 eyes and 24 mmHg in one eye. Conclusion：Leaking filtration blebs could be repaired by using preserved corneal graft without inducing elevation of intraocular pressure.
