Abstract. Purpose:To report a case of anterior ischemic optic neuropathy(AION)as the initial manifestation of microscopic polyangiitis(MPA). Case:An 87-year-old female presented with impaired vision in the right eye since 2 weeks before. Findings:Best-corrected visual acuity was no light perception right and 0.4 left. The right eye showed retarded light reaction and slight disc edema. Fluorescein angiography showed hyperfluorescence of the optic disc. MPA was suspected due to elevated blood sedimentation rate and CRP, and high value of myeloperoxidase anti-neutrophil cytoplasmic antibody(MPO-ANCA). The diagnosis was confirmed after she was found to have interstitial pneumonia and chronic nephritis. After treatment with systemic prednisolone and immunosuppressive agents, visual acuity in the right eye improved to 0.02 one month later. Her left eye remained unaffected until present. Conclusion:MPO-ANCA was elevated in this case of AION as the initial manifestation of MPA.
