眼窩筋炎として加療後に悪性リンパ腫へ進展した1例

A case of malignant lymphoma masquerading as orbital myositis

要約　64歳女性が2か月前からの左眼の上眼瞼腫脹と眼球突出で受診した。磁気共鳴画像検査（MRI）で左眼上直筋に肥厚があり，眼窩筋炎と診断した。プレドニゾロン内服で軽快したが，18か月後に激しい眼痛，頭痛，複視が左眼に生じた。MRIで上直筋を中心に眼窩内に広がる腫瘍性病変があり，生検を行った。免疫組織化学的検索と免疫グロブリンの遺伝子再構成の結果から，悪性リンパ腫と診断した。眼窩筋炎で初発し，悪性リンパ腫に発展した1例である。

Abstract.　A 64-year-old woman presented with swelling of upper eyelid and proptosis in her left eye since 2 months before. Magnetic resonance imaging（MRI）showed swollen superior rectus muscle leading to the diagnosis of orbital myositis. Clinical remission was obtained following systemic prednisolone. She developed severe ocular pain,headache and diplopia in her left eye 18 months later. A tumorous lesion was detected in the left orbit extending from the superior rectus muscle by MRI. The lesion was diagnosed as malignant lymphoma by immunohistochemistry and rearrangement analysis of immunoglobulin of biopsied specimen. This case illustrates that malignant lymphoma may develop from orbital myositis.
