眼部帯状ヘルペスの臨床像の現状と変遷

Changing clinical features and current status of herpes zoster ophthalmicus

眼部帯状ヘルペスの臨床像を自験例について解析した。角膜病変を伴う眼部帯状ヘルペスの症例数は，2004年までの5年間の16例と，1993年までの11年間の49例である。今回の16例の平均年齢は68歳で，男性11例，女性5例であった。今回の症例群を前の11年間のそれと比較するとき，男性が増加して平均年齢が有意に高齢化し，さらに実質型が減少し角膜ぶどう膜炎型と内皮型が増加していた。病型別の平均年齢は上皮型74歳，内皮型53歳，角膜ぶどう膜炎型71歳であった。16例中7例（44％）にパラシクロビルが投与されていた。高齢化と角膜病変が軽微な症例の増加が最近の眼部帯状ヘルペスの臨床的な特徴であった。

We reviewed the clinical features of herpes zoster ophthalmicus in our department. During 5 years from 2000 to 2004,16 cases were diagnosed with herpes zoster ophthalmicus with corneal involvement. The series comprised 11 males and 5 females. Their ages averaged 68 years. When compared with cases during 11 years through 1993,the present series was characterized by higher incidence of males,significant increase in age,decrease of stromal type,and increase in keratouveitis and epithelial type.
