We observed two eyes in two cases that developed pupillary occlusion after trabeculectomy. A 62-year-old female received trabeculectomy for acute closed-angle glaucoma 3 years before. She was referred to us for failing vision in the operated eye since 3 months. The corrected visual acuity was 0.03 due to pigmented pseudomembranous tissue in the pupillary area. Pupilloplasty resulted in recovery of vision to 0.6. Another 66-year-old female had received trabeculectomy for open-angle glaucoma 15 years before. The visual acuity in the operated eye decreased to 0.08 due to pigmented pseudomembranous tissue in the pupillary area. Pupilloplasty induced recovery of vision to 0.1. History of trabeculectomy and topical pilocarpine were common features in these 2 cases. We advocate due precaution in the use of pilocarpine in eyes with previous trabeculectomy.
