破格筋腱損傷を伴ったアキレス腱皮下断裂の1例

抄録：症例は39歳の女性で，バレーボール中受傷し，アキレス腱断裂の診断のもとに翌日縫合術を行った．断裂部を展開すると踵骨後方に停止する約20cm，幅10mm，厚さ2mmの扁平な腱が存在した．その近位側断端には筋が付着し，アキレス腱断裂に伴って筋腱移行部で断裂した破格筋腱と考え，アキレス腱断裂部を縫合後，その腱を用いて縫合部を補強した．
本例の破格筋腱は下腿三頭筋の破格が考えられ，第3腓腹筋やaccessory soleusの一種と推測した．
