エーラスダンロス症候群の治療経験―血管型の急死例を含めて

Ehlers-Danlos Syndrome in Orthopaedics. Our Experience, Including a Case of Sudden Death by a Patient with the Vascular Type

　背景：血管型エーラスダンロス症候群は，先天性内反足や先天性股関節脱臼で整形外科を初診することがあり，重大な合併症を来しうるので見逃せない疾患である．

　対象と方法：1996年から2013年までに当科でエーラスダンロス症候群と診断された5例について，初診時年齢，性別，初発症状，家族歴，当科で治療した疾患などを後ろ向きに調査し，血管型の可能性について改めて検討した．

　結果：死亡した1例を含む5例中3例で血管型の可能性が否定できなかった．

　まとめ：先天性内反足と先天性股関節脱臼の合併例は血管型の可能性があり，注意を要するものと思われた．

　Background：Patients with the vascular type of Ehlers-Danlos syndrome (EDS) are at high risk of serious complications, including massive bleeding, and are often seen in orthopaedic clinics for congenital clubfoot or developmental dysplasia of the hip.

　Methods：In this study we retrospectively reviewed the characteristics of 5 patients with EDS in whom the possibility of the vascular type was reconsidered.

　Outcome：The vascular type of EDS could not be ruled out in 3 cases, including in a sudden-death case as a result of aortic rupture.

　Conclusion：The simultaneous presence of congenital clubfoot and developmental dysplasia of the hip was considered to be a critical factor in the detection of the vascular type of EDS in this series.
