外傷性の橈骨動脈断裂に対して動脈吻合を行った血管型Ehlers-Danlos症候群の1例

Arterial Anastomosis for Radial Artery Laceration in Vascular-type Ehlers-Danlos Syndrome. Case Report

　症例は35歳の男性で，血管型Ehlers-Danlos症候群の既往があり，31歳時から34歳時にかけて動脈破裂に伴う四肢のコンパートメント症候群を3回発症していた．手関節部の切創による右橈骨動脈損傷に続き右前腕のコンパートメント症候群を発症したため，筋膜切開術と血管吻合術を施行した．血管型Ehlers-Danlos症候群例は治療に際して血管の脆弱性が問題となるが，四肢の外傷に伴う血管損傷であれば血管吻合を試みるべきだと考えられる．

　A 35-year-old male with vascular-type Ehlers-Danlos syndrome complained of right forearm pain after laceration of the radial artery.　He had a past history of a compartment syndrome in his limbs three times in the past four years.　We treated him by performing fasciotomy of the forearm and anastomosis of the radial artery.　Although ligation and embolization are recommended for the treatment of arterial ruptures in vascular-type Ehlers-Danlos syndrome because of the great fragility of the arterial wall, arterial anastomosis is worth trying in arterial laceration cases.
