　緒言：われわれは，比較的稀な上腕三頭筋皮下断裂の2例を経験したので報告する．

　症例：症例1は14歳の男児で，転倒して受傷し，受傷後8カ月時に当科を紹介受診した．陳旧性上腕三頭筋皮下断裂の診断で保存的に加療し，機能障害なく治癒した．症例2は56歳の男性で交通事故で受傷した．上腕三頭筋皮下断裂の診断で手術を行い，機能障害なく治癒した．

　考察：本外傷は見逃されやすく，診断にはflake signの同定が重要である．若年者や活動性の高い症例には手術療法を選択すべきであるが，それ以外の症例は慎重に手術適応を判断すべきである．

　Introduction：We report two cases of distal triceps rupture.

　Cases：Case 1：A 14-year-old boy fell in playing soccer, and 8 months after the injury came to our hospital complaining of limited motion of his left elbow joint.　He was diagnosed with an old distal triceps rupture, and conservative treatment resulted in restoration of normal elbow joint function.

　Case 2：A 56-year-old man was involved in a traffic accident and suffered a right distal triceps rupture.　Surgical treatment resulted in restoration of normal elbow joint function.

　Discussion：Distal triceps rupture is a relatively rare injury, and in some cases the diagnosis is missed.　In most cases, radiographs show the "flake sign" of an avulsed bony fragment from the olecranon, and it is important not to miss it.　Surgical treatment should be performed in young and active patients.　However, care must be exercised in determining whether surgical treatment is indicated in elderly patients.
