Introduction:We report two cases of distal triceps rupture.

Cases:Case 1:A 14-year-old boy fell in playing soccer, and 8 months after the injury came to our hospital complaining of limited motion of his left elbow joint. He was diagnosed with an old distal triceps rupture, and conservative treatment resulted in restoration of normal elbow joint function.

Case 2:A 56-year-old man was involved in a traffic accident and suffered a right distal triceps rupture. Surgical treatment resulted in restoration of normal elbow joint function.

Discussion:Distal triceps rupture is a relatively rare injury, and in some cases the diagnosis is missed. In most cases, radiographs show the "flake sign" of an avulsed bony fragment from the olecranon, and it is important not to miss it. Surgical treatment should be performed in young and active patients. However, care must be exercised in determining whether surgical treatment is indicated in elderly patients.
