 We analyzed 116 cases of spinal pseudarthrosis that had been treated surgically to clarify the initial treatment for osteoporotic spinal compression fractures and problems associated with it. Primary external fixation with a cast or a hard corset had been performed in only 11.2% of the cases. No external fixation had been performed in 44.6% of the patients who had compression fractures without any obvious cause of injury. An appropriate initial strategy that includes the diagosis of osteoporotic vertebral compression fractures should be devised to reduce the number of cases of spinal pseudoarthrosis that require surgical treatment.
