　抗cardiolipin抗体陽性，血小板減少や血清補体価低下を伴う，頸髄主体の再発性脊髄炎の症例を経験した。多発性硬化症類似の脱髄性病態と考えられた。再発時に頸部前屈により，仰臥位では両前腕尺側に，起坐位では両下肢にも不快な異常感覚が走り，Lhermitte徴候が体幹前屈の前後で変化した。頸部MRIで髄内の2レベルにgadoliniumで増強される病変が認められた。Lhermitte徴候の誘発に体幹前屈を追加することは有用であり，誘発手技の追加で段階的に増強するLhermitte徴候は，後索病変の多発を示す可能性がある。

A 36-year-old woman suffered from steroid-respon-sive relapsing myelitis associated with hypocomple-mentemia, thrombocytopenia, and anti-cardiolipin anti-body. At the second attack of paraplegia, neck flexion in a supine position induced uncomfortable dysesthe-sia radiating into the ulnar side of the bilateral fore-arms. Both truncal and neck flexion resulted in painful dysesthesia down into both lower limbs also.
