開頭生検が早期診断に有用であった頭蓋内結核腫の1例

A Case of Intracranial Tuberculoma Early Diagnosed by Open Brain Biopsy

　けいれんにて発症し，開頭生検にて早期診断された頭蓋内結核腫の31歳男性例を経験した。頭部CT上結核腫を疑わせる頭蓋内多発性腫瘤を認めたが，細菌検査をはじめとする諸検査では確定診断は得られなかった。第5病日目に画像上の増悪が認められたため，7病日目に右後頭葉の病変に対し開頭生検を施行。病理学的に頭蓋内結核腫の診断が得られた。ただちに抗結核剤の投与を開始し，開始後30日目にCT上の病変の拡大を認めたが，その後徐々に改善傾向を認め独歩にての退院となった。頭蓋内結核腫のCT診断は感度が高い半面，特異度は必ずしも高いと言えず偽陽性例も多い。また結核腫においては本症例にもみられた‘paradoxi-cal expansion’とよばれる一時的な増悪現象が認められることもあり，診断的治療も確実性に欠ける。本疾患においては可能な場合，開頭生検も検討する必要があると思われる。

A 31-years-old man suffered from cough, sputum, slight fever, and loss of consciousness. He was tenta-tively diagnosed as epilepsy, and was transferred to our hospital. Brain CT demonstrated multiple intracra-nial lesions, which were homogeneously enhanced. Chest X-ray revealed retiform shadow in the right middle lung field. Pulmonary tuberculosis and intra-cranial tuberculomas were suspected and bacterial cul-tures, PCR, smear examination of sputum, blood, CSF, gastric juice, and urine were repeated. But no evi-dence of tuberculosis was obtained.
