抗結核剤内服中に発症し，髄膜腫様の画像所見を呈した頭蓋内結核腫の1例

A Case of an Intracranial Tuberculoma, Mimicking Meningioma, that Developed during Treatment with Anti-tuberculous Agents

I．はじめに

　頭蓋内結核腫は，多くは肺結核からの血行性感染により主に脳底槽や脳実質内に形成される^2,12）．今回われわれは，抗結核剤内服中に発症し，画像・術中所見にて髄膜腫様を呈した頭蓋内結核腫の症例を経験したが，このような形状を呈するものは稀である．症例を提示するとともに，その形成機序，画像所見，鑑別点等を若干の文献的考察を加え報告する．

A 54-year old female patient presented with intracranial tuberculoma attached to the dura mater duringtreatment with anti-tuberculous agents. Computed tomography (CT) and magnetic resonance imaging(MRI) revealed a lesion resembling a cystic meningioma. A granulomatous tumor that adhered to the cere-bral cortex and dura was totally removed. Histological diagnosis was epithelioid granuloma. After removalof the tuberculoma and treatment with anti-tuberculous medications, the patient seemed to be cured, andregrowth of the tumor has not been observed to date. Tuberculoma mimicking meningioma is a very rarephenomenon. In the present study, we review intracranial tuberculomas and discuss their radiological fea-tures.
