　右前頭葉内側面を含む領域の腫瘍摘出術後に，反響言語を特徴とする非流暢性交叉性失語症を呈した症例を報告した。本症例は減弱型反響言語を呈しており，相手の発話の一部を取り込んだ後に自身の言語反応を続ける傾向があった。自分の名前や疑問詞など情報的価値のない内容を取り込むことも多かった。特徴的なことは，会話の相手の発話のみならず，周囲から聞こえてくる会話の一部も繰り返すことであった。反響言語には自覚があり，取り込んだ後には抵抗する様子を見せ，また繰り返しを自ら「邪魔なもの」と内省していた。しかし取り込みを禁止すると言語応答に遅延や誤りがみられるようになった。本症例の反響言語には，言語優位半球の前頭葉内側面損傷による環境依存性と同時に，非優位半球症状としての注意の転導性の亢進が関与するものと考えられ，患者自身はこの反響言語を発話開始の引き金として無意識に利用しているものと考えられた。

We report a right-handed woman, who developed anon-fluent aphasia after resection of astrocytoma(grade III) in the right medial frontal lobe. On admis-sion to the rehabilitation department, neurological ex-amination revealed mild left hemiparesis, hyperreflexiaon the left side and grasp reflex on the left hand.

Neuropsychologically she showed general inatten-tion, non-fluent aphasia, acalculia, constructional dis-ability, and mild buccofacial apraxia. No other apraxia,unilateral spatial neglect or extinction phenomenawere observed.
