　臨床的観察から筋萎縮性側索硬化症（ALS）の初期ALS像（障害経過の初めの段階）と報告した呼吸運動系先行麻痺型^10）の5剖検例を報告した。その神経病理像では，皮質脊髄路変性と下位運動ニューロンの脊髄および脳運動神経核運動ニューロンの従来から強調された変性に加え，全例に髄鞘の淡明化を含む脊髄前・側索変性と脊髄灰白質前角部，前交連部，脊髄灰白質周辺のグリオーゼと，3例に延髄網様体のグリオーゼが見られた。これらの変性部位は，臨床像から想定したHolstegeのFirst motor systemの前運動ニューロンと下位運動ニューロンに含まれ，ALS初期像の病理的表現と考えられた。この臨床・病理学的な初期ALS像からtotally locked-in state（TLS）の晩期像に至るALSの全体像（新しいALS観）と，今までの呼吸筋麻痺までのALS観との違いを検討した。

We reported five autopsied patients grouped as res-piratory motor paralysis preceded type of ALS (R-ALS) classified as one clinical type of ALS from the patients living beyond respiratory failure reported in the previous paper, in which ALS the ontogenetically new motor functions in the "first motor system" pro-posed by Holstege involved first.
