片側顔面痙攣で発症した小脳橋角部epithelial cystの1例

Cerebellopontine Angle Epithelial Cyst Presenting as Hemifacial Spasm

　片側顔面痙攣で発症した小脳橋角部epithelial cystの1例を報告する。症例は35歳男性で，2カ月前に発症した右顔面痙攣にて当科受診し，頭部CTにて右小脳橋角部に均一な低吸収域が認められた。手術用顕微鏡下に嚢胞壁は全摘され，術後顔面痙攣は消失した。本嚢胞の病理組織所見として，円柱もしくは扁平上皮様細胞の多層構造が認められた。さらに電顕所見としてmicrovilliを覆うcoating materialの証明に加えて，HE染色で認められた円柱もしくは扁平上皮様細胞が基底膜に連なってdesmosomeやtonofilamentを伴って認められた。特に後者の所見は呼吸・消化器系上皮に特異的な基底細胞の過形成に一致するものであり，coating materialの証明とともに本嚢胞が内胚葉由来であることを示唆する所見と考えられた。

A rare case of cerebellopontine angle epithelial cyst presenting as hemifacial spasm is reported. The patient was a 35-year-old man with right hemifacial spasm of 2 months duration. A preope-rative CT scan showed a well-demarcated low density area in the right cerebellopontine angle. The cyst was totally excised under an operating microscope. The postoperative course was unevent-ful, and the right hemifacial spasm resolved immedi-ately.

Histologically, the cyst wall was composed of a stratified epithelial lining composed of non-ciliated cuboidal or squamous cells.
