　特異な笑いを呈した前頭葉優位型葉性萎縮の1例を経験した。54歳，右利き女性で，活動性の低下と状況にそぐわない笑いを主訴に当大学精神神経科を受診した。本症例の笑いは，非特異的刺激に反応し出現する，感情の変化は伴わない，随意的に制御可能であると考えられる，という特徴を示した。笑いの表情筋電図では，持続時間は数秒間と，健常者とほぼ同じで，健常者ではほとんど収縮がみられないM．corrugatorsupercilii皺眉筋の筋放電の著しい増加がみられた。CT，MRIでは前頭葉中心に左に強い限局性の萎縮を認め，SPECTでは同部位の著明な血流低下が認められた。以上のことより，本症例の笑いはpathologicallaughing病的笑いなどの情動表出異常の範疇に属し，前頭葉の障害が笑いの発現に関与している可能性が示唆された。

We report a case of a Pick's disease (frontal predominant type) with unusual laughing. The pati-ent was a 54-year-old, right handed, female.

The patient began to show low activity and in-appropriate laughing at about 51 years. She did not speak at all and showed weird laughing with a voice "hi hi hi hi hi hi" frequently at first presentation. When asked some questions, she sometimes answer-ed with a nod. Almost all of her laughing did not match with the situation and seemed to lack a relationship between affective change and observed expressions.
