I．はじめに

　後頭動脈・椎骨動脈吻合（Occipital-Vertebral Anas—tomosis）は脳血管撮影を多数経験しているものにとつてそれほど珍しいものではない^{3,6,12,15,17,18）}。脳血管撮影により発見されるそのような吻合は，総頸動脈や椎骨動脈などの閉塞に伴う副行路のことが多いが^2,12,18），動脈の閉塞過程を有さない症例に発見されることも少なくない^3,17）。

　Schechter^12）は1000例の脳血管撮影において8症例のOccipital-Vertebral Anastomosisを経験している。そのうちの2例は頸部における椎骨動脈の狭窄を合併しており，第3例は内頸動脈結紮術後の血管撮影において発見され，第4例は内頸動脈および外頸動脈起始部の狭窄に合併していたが，残りの4症例には血管性病変を認めなかつた。

Four cases of the occipital-vertebral anastomosis, demonstrated on vertebral arteriogram, were re-ported ; three on the left side and one on the right side. The occlusion in the vertebral artery at the posterior end of the horizontal segment or the more distal segment of the vertebral artery was found in two cases and no occlusion involving the carotid and vertebral arteries was present in the remaining two cases.

The direct type of the occipital-vertebral anas-tomosis was found in all of the four cases in the atlanto-occipital space. The developmental mecha-nism of the anastomosis was explained based upon the authors' hypothesis on the embryonic develop-ment of the occipital artery.
