Practice of Microvascular Anastomosis

　近年Microsurgeryが発達し，脳神経外科領域でも広く応用され，脳動脈瘤，脳・脊髄動静脈奇形，脳腫瘍，脊髄腫瘍の手術に新しい時代を迎えたのは周知のとおりである．Microsurgeryがとくに必要とされる分野に，微小血管（とくに直径1mm前後）のanastomosisがある．これは脳神経外科医が是非とも身につけておかねばならぬtechniqueの一つである．これにより浅側頭動脈—中大脳動脈皮質枝吻合（Yasargil^1）），脳表主動脈のthromboembolectomyおよび脳動脈瘤あるいは外傷，腫瘍手術の際の不時の血管損傷の克服が可能となる．またこのtechniqueを神経縫合にも応用することができる．吻合手術後の成績を向上させるについては，練習につぐ練習がぜひとも必要である．この論文の目的は，著者が得た経験の中からmicrovascular anastomosisの種々のtechniqueの眼目を紹介し，さらにそれに伴う問題点を明らかにすることである．

It has become possible to repair an occluded cerebral vessel or to construct a new collateral circulation (extracranial intracranial anastomosis by Yaşargil) by using the microsurgical method. However, the microvascular anastomosis is difficult to perform and it can be rightly done only after reasonable and repeated exercises. The purpose of this paper is to report the details of the technique of anastomosis operation and to consider how to get a high percentage of patency in the anastomosis of vessels of about 1mm in diameter.
