はじめに

　脳側副循環路の中で，Occipital-Vertebral Anastomosisの認められる頻度は比較的少ないとされ，その報告の大部分が内頸動脈，椎骨動脈，又は鎖骨下動脈の閉塞，狭窄，及び偶然後頭動脈が穿刺された時に造影される筋枝間吻合枝であり，直接吻合枝の報告は数例にすぎない。我々は総頸動脈及び椎骨動脈撮影で内頸動脈，椎骨動脈，及び鎖骨下動脈に閉塞，又は狭窄がなかつたにも拘らず，直接吻合枝，筋枝間吻合枝が造影された1症例，及び3症例，計4症例を経験したので報告する。

Four cases of occipital-vertebral anastomosis vis-ualized on common carotid or vertebral angiographies by catheter technique were presented. None of the cases showed any occlusion or stenosis of internal carotid, vertebral, or subclavian artery. The inci-dence of this anastomosis in our institute was only 4 out of 2108 common carotid and 315 vertebral angiograms.

Case I. A 17-year-old male

Clinical diagnosis : cerebral palsy (Fig. 1. a, b, c)

The only anastomotic channel between occipital artery and vertebral artery with relative uniform diameter about 1.7mm was visualized angiographi-cally, but another branches could not recognized.

These findings might suggest direct anastomosis between the two arteries.

Case II. A 15-year-old male

Clinical diagnosis : Occipital neuralgia (Fig. 2. a, b, c)

Case III. A 12-year-old female

Clinical diagnosis : brain tumor (Fig. 3. a, b)

In both cases, common carotid artery and verte-bral artery were communicated through muscular branches.

Case IV. A 16-year-old male

Clinical diagnosis : head injury (Fig. 4. a, b)

Contrast media filled right occipital artery via muscular branches from right vertebral artery, though this anastomosis could not be demonstrated on right common carotid angiogram.
