There are two main theories as to the genesis of chronic subdural hematoma-namely the intradural theory and the subdural theory, but all of the theories still remain as mere speculations.

The common origin of chronic subdural hemato-ma is hemorrhage in or around the dura mater. From this view-point, Hirayama in our clinic in-vestigated the vascular changes in the innermost layer of dura mater, and found many histological changes, capillaries with ectasia, slight hemorrhage, capillarization etc., especially in males between the ages of 50 and 60 years, which is the age of pre-dilection for the development of chronic subdural hematoma. It is generaly said that the chronic subdural hematoma is more dominant in drinkers than non-drinkers and in males than females. It is also known that the high value of estrogen, which is especially responsible for liver dysfunction, de-velops the vascular spider. The vascular spider on the skin resembles, in appearance, to the innermost layer of dura mater.

Then taking into account these facts, the urinary estrogen excretion in cases of chronic subdural hematoma has been assayed. The value of estrogen was not only higher than normal range in the presents of the chronic subdural hematoma but the value continued to remain high even in follow-up studies after the osmotherapy, irrigation or subtotal removal of the hematoma (P<0. 001). Therefore, it was assumed that high value may not be the result produced by the mass effect of hematoma but rather an etiologic factor of chronic subdural hematoma.

Under this assumption, experimental investi-gations were carried out with rats in 3 different methods to determine the effect of estrogen ad-ministration on the dura mater. The results of the investigation suggested findings such as vas-cular hyperpermeability and/or stenosis, or occlusion of vessels in and around the dura mater, especially in male rats

The correlation between estrogen and genesis of chronic subdural hematoma was discussed in thispaper because estrogen value was regarded to be the fundamental cause of chronic subdural hematoma on the bases of above facts.
