生検筋で著明なTubular aggregatesを認めた甲状腺中毒性周期性四肢麻痺の1例

AN ELECTRON MICROSCOPIC FINDING IN MUSCLE OF THYROTOXIC PERIODIC PARALYSIS

緒言

　周期性四肢麻痺に関する報告は，1727年Musgraveによるものが最初とされている。以来，周期性四肢麻痺について多数の論文が見られるが，その本態はまだ明らかではない。1913年，Kitamura^1）が，初めて周期性四肢麻痺と甲状腺機能亢進症の合併に注目し，1926年Shinozaki^2）が同様の15例を報告して以来，主として木邦で両疾患の合併について幾多の論文が発表されている。しかし，周期性四肢麻痺の本態はもとより，四肢麻痺と甲状腺機能亢進症との合併や，これが日本人に多い理由など，未解決の点が多い。

　従来，家族性周期性四肢麻痺，あるいは甲状腺中毒性周期性四肢麻痺における麻痺筋の組織学的所見として，vacuolationが報告されているが^3）〜9），近年，電子顕微鏡（以下電顕と略）学の進歩により，筋微細構造の変化に関する報告も散見されるようになつた。

Electron microscopic studies were conducted with specimen oktained by muscle biopsy from a case of thyrotoxic periodic paralysis. Light microscopy with hematoxylin and eosin stain or with Masson's trichrome stain revealed no abnormal finding except for vacuole. Thick sections stained by toluidine blue showed some dense portions, which were electron microscopically disclosed to be tubular aggregates which may originate sarcoplasmic reti-culum.

The tubules were almost in parallel with myo-fibrill measuring 58-65 mit in diameter and 70-80 Å in the thickness of limiting membrane and contain-ing the core of 26-39 mμ. Some of the tubules were enlarged to measure 0.1-0.7 μ in diameter. Besides, dilation of terminal cisternae and increase of glycogen granules were noted but degeneration of myofilament was not observed.
