うつ状態に対するChlordiazepoxide （Contol）, Isocarboxazid （Enerzer）併用療法

COMBINED USE OF CHLORDIAZEPOXIDE (CONTOL) AND ISOCARBOXAZID (ENERZER) IN DEPRESSIVE STATE

I．はじめに

　最近，抗うつ薬が多数あらわれているが，それらは作用機序の上から大さくわけると^1），1） Neu—roleptics, 2） Stimulants,3） Energizer，　4） Thymolepticsに分類することができ，主な抗うつ薬を表示すると第1図のようになる。これらに属すさまざまの抗うつ薬が，すべてのうつ状態に同じような効果をもつものでないことは，今までの臨床上の経験から周知の事実である。すなわち，抗うつ薬にはある程度その薬物に特有な目標症状というものがあり，ひとつの薬物だけでうつ状態のすべての症状をカバーして治療することはできない。主な抗うつ薬について，うつ状態の症状に対する作用のおおまかな程度をあらわしてみると，第1表のようになる。

　このようにみてくると，抗うつ薬の一種を単独で使用するより，同時か継時かの問題は別として，2種，あるいはそれ以上の抗うつ薬を併用することが，臨床上必要であることがうなずかれてくる。どのような抗うつ薬を併用するかは臨床上妙味のあるところであるが，第1表からも考えてIsocarboxazidとChlordiazepoxideの併用が理論的にすぐれたものであるように思われる。もちろん，他にもいろいろな組みあわせは考えられる。わたくしたちはさきに，LevomepromazineとImipramineを併用してそれぞれを単独投与した場合より，よりすぐれた効果をあげることができた^2）。そこでこんどはChlordiazepoxideとIsocarboxazidの併用の臨床実験を行なてみたのである。

In 42 cases of endogenous, involutional, neur-otic and reactive depressions, treatment was carried out with Contol combined with Enerzer (26 cases), of Contol alone (16 cases). Better results were obtained by combined use of Contol and Ener zer compared with these agen-ts used alone or other antidepressive agents.

Generally, anxiety, irritation and insomnia disappeared first and then desparate and de-pressive feelings were improved by Contol while decrease in volition and spontaneity was improved by Enerzer. The effect became apparent rapidly, within three days in 75 per cent of the cases.

Side effects were noted in 34 of 42 cases. Feeling of imbalance, and dizziness were most frequent and edema in the face and extremi-ties, drowsiness and orthostatic hypotension followed.
