An autopsy case of a 9 years old girl was reported, whose clinical features were extre-mely similar to those of syringomyclia over 5 years. The postmortem examination reve-aled a large tumor mass involved the medulla at the level of the oliva bulging posterio-superiorly, measuring 3.0×2.5×2.0 cm. The tumor was cystic and extended to the spinal cord down to the D5 segment. There was a cystic cavity in the posterior tract of the C6. Microscopically, principal part of the tumor consisted of bipolar fibrillated cells, which were characteristic of pilocytic type of astrocytoma. The clinical symptoms arised from the tumor itself, not directly from cystic cavity resulted from degeneration and lique-faction of the tumor cells.
