対麻痺を呈したChondrodystrophia fetalisの1例

A CASE OF CHONDRODYSTROPHIA FETALIS ASSOCIATED WlTH PARAPLEGIA

緒言

　Chondrodystrophia fetalisは1892年Kau-fmanによつて名附けられ，軟骨の発育障害及び軟骨内化骨機転の早期停止を来す疾患で，胎生期に始まり，躯幹の矮小，四肢の短縮，頭蓋の変形，鞍鼻，三叉手，仙骨の変形等を特長として居り，Kaufman^1）はこれを三型に分類して，1） Chon-drodystrophia hypoplastica 2） Chondrody-strophia malacia 3） Chondrodystrophia hy-perplasticaとしているが，各型の混在や移行型が多く明確な例は少い。通常胎生6〜9カ月で死亡するか出産後間もなく死亡するので，一般には妊娠，或は出産時や乳児，又は小児期に注意がはらわれ，この時期における報告は多いが，成人のChondrodystrophiafetalisについての記載は割合に少い。従つて成人に於ける報告は主として整形外科的問題が多く，その多くは特異な骨異常から来る症状が主として取りあげられ，四肢の短小，即ち長管骨の肥大，骨端の腫大，頭蓋の変形等である。一方脊椎の変化は一般に四肢ほど著明でないため看過される事が多いが，椎弓椎体の化骨機能障害により早期に個々の椎体の癒合がおこり，椎体の縮少，変形が見られ又全脊椎管の狭少，殊に前後径の狭少が見られると云われている。

A case of atypical achondroplasia associa-ted with paraplegia was reported. 27 years old unmaried female developed to spastic paraplegia with gradual onset at age of 25. Her face, body and extremities showed aty-pical chondrodystrophia fetalis. Neurological and myelographic examination revealed defi-nite pressure syndrome at the level of dorsal 9 spine. Laminectomy relieved her symptom. The cause of this paraplegia based on not only narrowing of spinal canal, deformity of in-tervertebral disk but also on some metabolic changes suggesting spondylitis dorformans or arthritis.
