  • Look Inside

In this report we have described three cases in which the spinal root of trigeminal nerve (SpV.) was affected by lesion of the medulla oblongata. We have experienced some intere-sting facts such as follows.

1) Sensory disturbances due to lesion of SpV. We have determined the normal sensory thre-shold in the face by using the methods of von Frey's Hair, sensory chronaxia, Hardy's appar-atus and compass two points discrimination. We recognized that the following symptoms in the affected side of face are shown by the injury of SpV.

a) pain and temperature sensation was markedly impaired.

b) tactile discrimination was normal.

c) tactile sense was normal or only sli- ghtly disturbed.

2) Moreover,we have observed three cases which showed peripheral trigeminal lesion by tumor of middle cranial fossa or herpes zoster and so forth. In these cases, the sensation of all modalities was impaired, in the region oftrigeminal branches affected. We found that the sensory disturbances by peripheral trigeminal involvement were different from that of SpV. in nature.

3) From the previous reports regarding to occlusion of the posterior inferior cerebellar artery sensory disturbances should be classified into four groups. In the majority of these cases, sensory disturbances are shown in the face of affected side and in the contralateral half of body (except face.).

4) In our previous report we had discussedthe type of sensory disturbances caused by les-ion of the lower pons which showed infranucl-ear facial palsy and proposed to classify them into six types. The curved course of the root fibres of the Nucleus Ambiguus is anatomically similar to those from the facial Nucleus. From this anatomical point of view, we attempt to classify the sensory disturbances caused by such lesions of medulla oblongata as involved the root fibres from the Nucleus Ambiguus into six types as in lesions of the lower pons.
