側脳室「メニンギオーム」に就いて

MENINGIOMAS OF THE LATERIAL VENTRICLES OF THE BRAIN.

緒言

　以前に渡邊^7）が報告せる多發性「メニンギオーム」の如き稀有例を除けば，「メニンギオーム」は通常單一性に發生し，然も腦硬膜と癒着し或は之に浸潤して行くものであるとの考え方が常識である。腦硬膜と癒着する一方，その發生原基であるarachnoid remnantの上皮樣細胞が腫瘍を構成している所から，「メニンギオーム」は曾つてdural endotheliomaなる名稱で久しく呼ばれたものである。此のarachnoid remnant cellsは正常の腦蜘網膜にも存し，殊に「パキオニ」氏肉芽のある部位に多い爲，斯かるものより生ずる腫瘍が一定の好發部位を示すのは周知の通りである。尤も蜘網膜組織のない腦硬膜にも生ずると言われる（Elsberg）。即，所謂displaced mesenchymal　cellsから生じ，爲に腦硬膜自體から發生し得ると言われるが，何れにせよ腦硬膜と癒着した形を取り易いわけである。然し乍ら腦硬膜とは全く無關係に腦皮質下に完全に埋沒された「メニンギオーム」，或は腦室内に生じた「メニンギオーム」の報告も，後に掲げる諸文献に見出し得る。

　Globus^4）は個體發生學的見地よりskeletone-　ural intertissue （Hallerstein）からprimitive　meninxが作られ，之よりecto-and endo-meni-nxが出來るが，之等は更に分化して前者よりは骨膜と腦硬膜とが，後者よりは蜘網膜と腦柔膜とが形成されると言い，此のprimitive meninx系から生ずる腫瘍即ち「メニンギオーム」を8型に簡單化して分類したが，要するに此の系統の組織の在る所は何處を問わず「メニンギオーム」發生の可能性が存在する筈である。

　Cushing and Eisenhardt^2）は彼等の優れた尨大な「モノグラフ」Meningiomasの中に一章（Ⅶ．　Meningiomas Without Dural Attachment）を割いてplexus-meningiomas, lateral tumors　of the velum，等々，腦硬膜とは關係のない「メニンギオーム」の事を記載している。此の種の腫瘍はplexus chorioideus或はvelumよりのvascular fringesより生じ來るものならんと考えられている。元々plexus chorioideusにしろ，velumにしろ，組織發生學的には腦蜘網膜の連續で腦室腔とはneuroepithelial ependymal）cellsを蔽つて隔つているに過ぎない。それで之等より生ずる「メニンギオーム」の存在は肯定し得，亦斯樣な腫瘍は全く腦室内にあつたり，或は一部腦室内，一部は腦實質内に埋れて外面よりは見えず，勿論腦硬膜とは關係がない事も想像に難くないわけである。

　現在迄に，手術或は剖検並びに組織學的検査に依り確證せられた京大荒木外科頭蓋内腫瘍例の中に52例の「メニンギオーム」があるが，此の中，腫瘍が全く側腦室内にあつたもの2例，一部側腦室内に存し他部は腦實質内に埋沒されたもの2例加之一部側腦室内，大部分は腦實質内，極少部は腦外表に現われていたもの1例，計5例を得ているので至極簡略に其等症例を記述報告し，併せて考察を加えて見たい。

Five cases of meningiomas of the lateral ventricles are reported in this paper. And a brief survey of the frequency of such tumors as compared with all intraventricular tumors of the lateral ventricles is given (11. 67—57. 32%). As regards the lateralization, the men-ingiomas in question occur, at least, twice more frequently on the left side than on the right side; the confidence interval of the ratio between the right-sided and the left-sided being 8.37—47.55%. Symptomas of such tumors are not uniform and even the ventriculography may fail to yield any information about the tumor. But when a tumor of the lateral ventricle is suspected, particularly on the left side, a meningioma should be considered as a diagnosis of likelihood. Given a correct preoperative diagnosis, this tumor would be a favorable surgical lesion. Histopathologically the tumor may take any type common to intracranial meningiomas of other localization.
