從來よりも簡易迅速に脳室穿刺4)を行い得ないものか—現今行われている經眼窩前頭葉切離術(transorbltal lobotomy1)2))のように眼窩を經由すれば穿頭術を行う必要がないであろう。このような見地から,我々は經眼窩脳室穿刺並にその氣脳撮影の問題を檢討してみた。そして,特殊な穿刺針を考案作製し,試行してみたところ,かなり満足すべき結果を得た。次にその大略を報告する。
精神外科としてのtransorbital lobotomyの實施に當つては,脳室損傷は絶對に防止しなければならない。この爲には脳室穿刺により脳室の位置を知る必要があるが,これは從来のlobotomeでは不可能であつた。脳室穿刺も可能なlobotomeの設計,これが我々の研究の主要眼目でもあつたが,この目的に適うように穿刺針は外筒と内針から成つている。内針を外筒におさめると,それ自體がlobotomeともなり,前頭葉切離術の施行が可能となる(第1圖A參照)。
In order to performe the ventricle puncture more easily, the authors have attempted to apply the technique for transorbital lobotomy (Freeman and Watts), and then, certain succe-sses have been obtained. As the needle for puncture, the one consisting of tube and sty-let was newly designed by the authors, which is shown in Fig. 1. This needle is also appli-cable for transorbital lobotomy as the loboto-me having some long points. The direction of insertion of the needle is important and it is shown as heavy lines Fig. 2, which were deter-mined after criteria of pneumoencephalograms of various cases. The technique for insertion of the needle and that for pneumo-ventricu-lography were mentioned, in addition to de-monstration of thus ventriculograms made (Fig. 3). That the transorbital ventricle pun-cture, of which idea seemed to be not a new one, and the transorbital pneumoventriculogra-phy seemed possible to be put into practice and also, its method seemed to be definite by our contribution, had been described. In fact, it was usually possible to manipulate the puncture and the air-liquid-exchange within few minutes.
