分裂病一卵性双生児の一致症例—破瓜型と単純型

A Case of Monozygotic Twins, both of whom Developed Schizophrenics: Hebephrenic and Simple Type

　抄録　症候論的に著明な差異を呈し，それぞれ破瓜型（A），単純型（B）と診断される分裂病一卵性双生児症例につき報告した。Aは10代より徐々に発病し，一時期幻覚妄想を呈し，現在は情意鈍麻を主とした慢性様態にある。Bは放浪生活を経て，20代後半より徐徐に適応不全を示し，明白な分裂病症状は認められないものの，人格の単純化が顕著な状態にある。この両者について比較検討し，症候論的差異と単純型分裂病の疾病構造に関して若干の考察を加えた。両者には出生時体重（A＜B），養育者との関係，病前性格などに差異が見いだせるが，Aのより早期の発病には，彼の無防備な構えが大きな要因として関与していることが推察された。一方BはAを自分の潜在的可能性として捉え，状況と関わることを回避することによって明白な発病を免れたが，適応レベルの低下とともに人格の単純化が進行した。こうした定着しない生の様式と，一面的思考による人格の再組織化が，単純型分裂病の様態を規定する要因として考察された。

　In this paper, the authors present a case of monozygotic twins, both of whom became schizophrenics, but who showed symptomatic differences. One (A) was hebephrenic and the other (B) was the simple type of schizophrenic. We compared their deve'opmental history and character traits, and discussed the psychopathological structure of the simple type of schizophrenia.

　In case A the disease began insidiously in early adolescence, and gradually progressed. At 39 years of age he was admitted to a psychiatric hospital in a hallucinatory-delusional state, which continued for about one year and a half. After this acute state subsided he was discharged. But his emotional blunting and social isolation has persisted until now.
