The patient was a 57-year-old woman with effortangina pectoris.99mTc-tetrofosmin myocardial SPECTshowed reduced uptake in the anterior, septal and apicalwall. Although right coronary arteriography findingswere normal, left coronary arteriography revealedmyocardial bridging in the mid-segment of the leftanterior descending coronary artery. These findingssuggest that, in this patient, myocardial ischemia mightbe explained as having been caused by myocardialbridge. Because of this indication, she was treated withCa-antagonist, β-blocker and antiplatelet agent. How-ever, these medications induced severe angina pectoris.Intravascular ultrasound revealed intimal proliferationin the myocardial bridge, so a stent was implanted inthis region. After stent implantation, myocardial bridg-ing on the coronary arteriography disappeared, chestsymptoms improved, and 99mTc-tetrofosmin myocardialSPECT showed normal uptake. These findings suggestthat stent implantation may be a useful treatment incase of myocardial bridging with resistant anginapectoris.
