An 82-year-old woman with continuous heart mur-mur who was evaluated by cardiac catheterization and coronary angiography revealed coronary artery fistula (CAF) originating from both coronary arteries. It was associated with giant saccular aneurysms and both arteries drained to the main pulmonary artery. Recent advances in medical technology enable closer and clear-er observation of coronary blood flow by echocardiogra-ahy.

The follow-up study by echocardiography also showed outlets of CAF draining to the outflow tract of the right ventricle. These findings were obscure in coro-aary angiography even retrospectively.

These results suggest that observation of coronary arteries not only by coronary angiography but also by echocardiogram or other techniques gives us more use-ful and exact information of CAF, and it might affect classification of CAF.
