左冠動脈用三次元変形Judkinsカテーテルの考案とその有用性

Evaluation of the Usefulness of a Three-dimensional Modified Design of Judkins Catheter for Left Coronary Angiography

　左冠動脈造影において，通常のJudkinsカテーテルでは挿入困難な症例に対する，第二選択のカテーテル（lntervec製type—岩国カテーテル^®）を考案した．これは左冠動脈用Judkinsカテーテルの第2ループを30後方にひねって三次元的に固定させたもので，通常の操作で容易に後上方に偏位した冠動脈口に到達する．冠動脈造影393例（延べ515回）のうち，通常の左冠動脈用Judkinsカテーテルを原則として2種以上使用しても挿入困難な症例29例に対し，本カテーテルを試用した．このうち過去2年以内に通常のカテーテルで挿入可能であったもの4例をB群，それ以外の25例をA群とした．A群は25例中22例に，B群は全例に挿入可能であった．総造影成功率は93.6％（368/393）から，99.2％（390/393）に上昇した．また，本カテーテルによる合併症は特に認めなかった．本カテーテルは第二選択カテーテルとして有用である．

We designed a new type of modified Judkins catheter. named Intervec Iwakuni-Type Cathetep^®. This catheter is twisted 30° posterior at the second loop of the ordinary Judkin's catheter for the LCA. We performed 515 catheterizations for 393 patients from Jan. 1993 to Apr. 1994. We used this new catheter for 29 cases in which we couldn't insert more than two ordinary Judkins catheters into the LCA. In these cases, we had been able to insert an ordinary Judkins catheter in 4 cases within the last 2 years (group B). We were never successful with an ordinary Judkins catheter in the other 25 cases (group A). In group A, we succeeded in 22 cases (88.0%). In group B, all 4 cases were successful. The overall success rate of the Iwakuni-Type Catheterl^® is 89.7% (26/29). Use of our new catheter raised our CAG success rate from 93.6% (368/393) to 99. 2% (390/393). We have not encountered any significant complications. The Iwakuni-Type Catheter is useful as a back-up second choice for Judkin's technique LCA angiography.
