　慢性肺高血圧症はその病因の如何にかかわらず予後不良な疾患とされており，本症の増悪に際しては有効な治療法がないのが現状である．なかでも，浮腫，肝腫大，胸水の出現増加によって呼吸困難に陥った状態に対して一時的に除水を行う適応はあるが，一般的な治療法とは考えられていない．

　われわれは，1990年6月このような症例に対し体外限外濾過（ECUM）を施行した．ECUMによって症状の改善が得られたため継続的に行い，また無尿となってからは，血液透析に変更してほぼ2カ月間にわたる重篤な状態からの離脱に成功した．

No matter what the cause may be, the prognosis of chronic pulmonary hypertension is grave, and we have no effective treatment for deteriorated cases.

In June, 1990, we performed the extra corporeal ultrafiltration method (ECUM) for a critical case of chronic pulmonary hypertension in which the patient suffered from progressive dyspnea and abdominal full-ness. By this method, his symptoms were improved. So, we performed it every day. When he had no micturi-tion, hemodialysis was performed instead of ECUM. For two months, he has had good micturition and has been in a good physical state.
