自然発作による左右同時冠攣縮を冠動脈造影にて確認しえた異型狭心症の1例

Successful Documentation by Coronary Angiography of Spontaneous Simultaneous Multivessel Coronary Spasm in a Variant Angina Patient : A case report

　自然発作による左右冠動脈の同時攣縮を冠動脈造影にて確認しえた異型狭心症の1例を経験したので報告する．症例は59歳，女性．安静兼労作時の胸痛を主訴に当院入院となった．入院時および運動負荷心電図は有意のST-T変化は認めなかった．冠動脈造影では明らかな狭窄は認めなかったが，その後胸痛を訴え，心電図では前胸部誘導および下壁誘導にてST上昇を認めた．冠動脈造影では左右冠動脈近位部に90％以上の攣縮を認めた．ISDNを冠注したものの心拍数30／分の徐脈，Electro-mechanical dissociationを起こし，一時的に意識消失したが，カテコラミン投与，心マッサージにより回復した．多枝攣縮は致死的不整脈やElectro-mechanical dissociationを起こしやすいといわれているが，今回の症例は自然発作での多枝攣縮を冠動脈造影で証明でき，非常に興味ある1例と考えられた．

A 59-year old female who complained of chest dis-comfort was admitted to our hospital. Electrocardio-gram (ECG) on admission and treadmill exercise test showed negative for ischemia. She underwent coronary arteriography. Initial angiography showed there was no significant coronary arterial stenosis. However, when we were preparing the spasm provocation test, she complained of the same kind of chest discomfort as she had felt before. We found that ST segment was ele-vated in both the anterior and inferior leads on the ECG.
