A 43-year-old male was admitted from January, 1993 because of chest pain early in the morning. During the period he Was being treated, complete A-V block anc ST elevation with chest pain occurred. In cardiac angiography, no significant stenotic lesion was revealed although the left coronary artery originated from the right sinus of the Valsalva, and the left main coronary artery transversed between the aorta and the maintrunk of the pulmonary artery. There are very fewcases with a combination of aberrant left coronary artery origin from the aorta and vasospastic angina pectoris. It was suggested that there was a possibility that sudden unexpected death could be caused by vasospasm in this case.
