　症例は43歳，男性．1991年1月頃より早朝安静時の胸痛を自覚．1992年1月より発作が頻回に出現するため精査・加療目的で1993年1月に入院した．ホルター心電図で夜間安静時の胸痛を伴うST上昇と完全房室ブロックを認めた．冠動脈造影では有意狭窄を認めず，左冠動脈が右バルサルバ洞から起始し，左冠動脈主幹部は大動脈と肺動脈幹の間を走行していた．異型狭心症を合併した左冠動脈起始異常は成人例では極めて稀で，かつ本症例では冠攣縮が死亡原因となる可能性が示唆されたことから，冠動脈起始異常例では異型狭心症の合併を念頭に置いた対応が必要である．

A 43-year-old male was admitted from January, 1993 because of chest pain early in the morning. During the period he Was being treated, complete A-V block anc ST elevation with chest pain occurred. In cardiac angiography, no significant stenotic lesion was revealed although the left coronary artery originated from the right sinus of the Valsalva, and the left main coronary artery transversed between the aorta and the maintrunk of the pulmonary artery. There are very fewcases with a combination of aberrant left coronary artery origin from the aorta and vasospastic angina pectoris. It was suggested that there was a possibility that sudden unexpected death could be caused by vasospasm in this case.
