重症筋無力症と胸腺—その病態と臨床に関する考察

Clinicopathological studies on myasthenia gravis and thymus

　重症筋無力症（MG）は，伸経筋接合部のシナプス後膜に存在するアセチルコリン受容体（AChR）に対する自己抗体（抗AChR抗体）によって，神経筋伝達が障害されて発症することが明らかになっている。すなわち，AChRに対する抗体依存性の自己免疫機序によりAChRの破壊や数の減少が惹起され，その結果，神経筋伝達障害をきたし，それが臨床的には筋力低下・易疲労性となって現れてくる^1）。

　また，MGの発症と病態に対して，古くから胸腺の関与が指摘されており，事実，MGは胸腺腫もしくは胸腺異常を高率に合併し，手術で胸腺腫および胸腺を摘出することにより症状寛解がみられる。最近では，胸腺が自己抗体産生に関与していることも証明されている^2）。すなわち，MG患者の胸腺にはリンパ濾胞形成を伴うことが多く，リンパ濾胞を形成するリンパ球の主体がB細胞であり，このB細胞の大半が抗AChR抗体産生能を有することも立証されている^2）。

In this study, we investigated the relationship between myasthenia gravis and thymus. We trans-planted the thymus with myasthenia gravis in nude mice and examined the possibility of occurrence of myasthenia gravis in nude mice. In the electrical testing we observed waning phenomenon in nude mice, but we did not observe the symptom of my-asthenia gravis in nude mice. The transplanted thymus changed into degenerations and necrosis.

On the other hand, we injected the plasma of my-asthenia gravis into nude mice. But in the electrical testing, we did not observe waning phenomenon in nude mice.

Prethymectomy plasma exchange reduced anti-acethylcholine receptor antibody titer and occurrence of postoperative crisis. We concluded that prethy-mectomy plasme exchange was effective for the post-thymectomy management.
