We studied consecutive five patients with ven-tricular septal rupture (VSR) following acute myocardial infarction by ultrasonographic methods (two-dimensional, contrast and pulsed Doppler echocardiography) in previous two years. In four patients with a serial examination, two-dimensional echocardiography allowed direct visualization of VSR. Contrast echocardiography revealed VSR only in one patient by a right to left shuntingcontrast flow. Pulsed Doppler echocardiography demonstrated turbulent (pattern A) or laminar (pattern B) shunting flow at the right ventricle in 3 cases. Furthermore, in two cases, the shunting flow signal was demonstrated when the defect was unclear by two-dimensional echocardiography alone. Surgical correction was performed in four patients. The location was in agreement with VSRlocation by two-dimensional and pulsed. Doppler echocardiography in each And all four patients survived.

We conclude that a combined use of two-dimen-sional and pulsed Doppler echocardiography allows accurate detection and localization of VSR and enables early corrective surgery.
