Right and left ventricular volumes were calculated in 31 patients with postoperative tetralogy of Fallot in order to disclose the effects of residual pulmonary stenosis and insufficiency (PSI) and of extent of the right ventricular (RV) outflow patch on the ventricular volume characteristics.

No relation was observed between RV systolic pressure and RV enddiastolic volume (EDV), RV ejection fraction (EF), LVEDV, and LVEF.

As the severity of PI was expressed by the ratio of RV systolic output by volume study to by thermodilution technique, this ratio was positively correlated with RVEDV. Furthermore, the ratio was larger in a group with mild or no residual PS than in a group with significant PS, indicating that the residual PS was effective against PI.

Regardless of extent of the RV outflow patch, RVEDV was larger than normal. RVEF was depressed below normal in patients with transannular patch. In this group, RV outflow contracted poorly with the mean EF of 0.44, when RV was divided into outflow portion and sinus portion.
