心房ペースメーカー植え込みによる薬物抵抗性心室頻拍の治療例

A case of refractory ventricular tachycardia treated with permanent atrial pecemaker implantation

　近年ヒス束心電図（HBE），心房早期刺激法の応用にて発作性上室性頻拍症（PSVT）の電気生理学的検討^1）がなされており，PSVTの機序としてreentry説とecto—pic-firing説があったがその多くはreentry説で説明されるようになった。一方心室性頻拍症（VT）についてもPSVT例同様心室早期刺激法が用いられ，頻拍の誘発，停止^2,5）が試みられている。VTについてもrecntry説とectopic説があり，更にreentryはmacro-reentry型^3,6）とmicro-reentry型^6,7）にわけられる。現在VTの機序としてmicro-reentry^7,8）説が主体をしめているかに見えるが，なおこれらを臨床的に多くのVT例で証明する事は容易ではない。PSVT,VTの治療に際して種種の薬物が使用され多くの例では充分な効果が得られているが，一部症例は薬物抵抗性でその治療に困難を感じる事がしばしばである。これら薬物抵抗性のPSVT,VTに対しは近年外科的処置がとられるようになった。その1つは直接不整脈の発生部位を切りとるか，reentryの経路を切断する事^3）にある。他の1つは心臓ペースメーカー治療にある。即ちPSVTに対しては誘導型の心房ペースメーカーが植えこまれ，VTに対しても心房または心室ペーシング^9,13）が行われ，心室粗動（VF），心室細動（Vfに対しては植え込み型除細動装置^14）の開発がなされている。

Recent years, His bundle electrogram and extra-stimulus method have been applied to analysis of ventricular tachycardia (V.T.) and its mechanisms were made clearer to offer better treatment. Various kinds of drugs are used to treat V.T. and in most of the cases they are enough effective. But in some of the cases V.T. is drug-resistant. The treat-ment is difficult in such cases. Recently, two kinds of surgical treatment are applied to the drug-resistant V.T. One is to directly excise either the part of the origin of arrhythmia or re-entrant circuit. The other is pacemaker implantation.
