　56歳，男性の拡張型心筋症例にみられた持続性心室頻拍の発作予防に対し，各種抗不整脈薬の単独または併用療法が無効で，有効薬も顆粒球減少や心不全増悪のため継続使用できなかった．発作停止に有効であったプロプラノロール静注後に出現した完全房室ブロックに対し，体外性ペーシングを施行したところ，このペースメーカーを用いたオーバードライブまたはアンダードライブペーシング法により持続性心室頻拍発作が容易に停止し，ペーシングにより頻拍周期も促進しないため，抗頻拍ペースメーカーの植え込みを行った．植え込み後，心機能が改善したためメトプロロール少量漸増療法を施行し，4年を経過しているが，ペースメーカーによる心機能や心室頻拍の増悪はない．本例は抗頻拍ペースメーカー治療が両者のコントロールに有用であったと考えられる．

A 56-year-old male with dilated cardiomyopathy was admitted for treatment of sustained ventricular ta-chycardia (SVT) and heart failure. Single or combina-tion use of various antiarrhythmic agents was ineffective for the prophylaxis of the SVT. Some antiarrhythmic agents induced granulocytopenia or even aggravated heart failure. Intravenous propranolol successfully terminated the SVTs. However, tempo-rary pacing was required for the treatment of complete atrio-ventricular block induced by propranolol. There after, SVT could easily be terminated by overdrive ventricular pacing.
