Pure-Mahaim束伝導による心室早期興奮例について

Ventricular preexcitation due to pure-Mahaim conduction

　一般に心室早期興奮をしめす副伝導路^1,2）としてはKent束，Mahaim束，James束がある。心電図上Kent束伝導型としてはPQ短縮とΔ波を伴う幅広いQRS波を呈し，Mahaim束伝導型としてはPQ短縮を伴わずΔ波を有するQRS波として，またJames束伝導型としてはPQ短縮を伴う正常QRS波形例として知られている^1,2）。中でもKent束，James束については多くの電気生理学的検討^3,4,5）がなされている。一方Mahaim束の存在が電気生理学的検索によりたしかめられた報告はそのほとんどがKent束や，James束との合併例^6〜10）であり，pure Mahaim束伝導例での報告はなお少ない^10,11,12）。今日われわれは2例の心室早期興奮例につき電気生理学的検討を加えpure-Mahaim束伝導であることを確認しえたのでここに若干の文献的考察を加え報告する。

Electrophysiological studies were performed in two patients with ventricular preexcitation due to pure-Mahaim conduction. Case 1 was a 21-year-old woman and the electrocardiogram (ECG) revealed normal sinus rhythm with a P-R interval of 0.12 sec and a small delta wave at the onset of the QRS complex.
