数年来重症発作に著効を示してきたhydrocortisone sodium succinateに対しアナフィラキシー様反応を呈したアスピリン喘息の1症例

Anaphylaxis-like reaction following intravenous injection of hydrocortisone sodium succinate, which had been effective for several years, in aspirin sensitive asthmatic patient

　気管支喘息重症発作の治療にあたり，ステロイド剤（特にhydrocortisone）の静脈内投与は，その有効性と救命的な意味から最も重要な治療法の一つである^1）〜3）。また，幸いにも重篤な副作用はきわめてまれで，安全に使用できる薬剤とされてきた。しかし，1974年Men—delson等の報告^4）以来ステロイド剤の静脈内投与にて重症発作が誘発されたという報告が散見されるようになり，ステロイド剤に対する過敏性が注目されている^6）。著者等も，重症発作に対し数年来使用し，著効を得ていたhydrocortisone sodium succinateに対し重篤な呼吸困難発作が惹起されたアスピリン喘息症例の1例を経験したので報告する。

A 36 year old male patient who was aspirin sensitive asthmatic, developed a severe bron-chial reaction and resulted in respiratory arrest following intravenous injection of hydro-cortisone, which had been effective for therapy of status asthmaticus over 3 years.

Positive skin test reactions were obtained to all hydrocortisone (Saxizon, Solu Cortef, Hydro-cortone)
